Who can obtain a family record book?
- The bride and groom at the wedding ceremony
- The unmarried parent(s) after the birth of the 1st child, insofar as parentage in respect of the child has been established
- The person or persons who have adopted a child
Where can I request a family record book?
At the local town hall :
- for the bride and groom
- birth certificate of 1st child for unmarried parents
- of the place of residence of the persons concerned in all cases
When can I request a second copy of the family record book?
- In the event of loss or theft of the passbook (submit a report to the police or a certificate of loss and proof of address)
- In the event of divorce, legal separation, de facto separation or disagreement
- When there is a change in the parentage or name of the persons appearing in the passbook
Delivery of family record books
- At the town hall on presentation of an identity document

The use of a family record book that is incomplete or has become inaccurate due to changes in the status of the persons concerned renders the person responsible liable to criminal prosecution.
